Overview

A collective of 20 adult literacy non-profits regularly need to refer applicants between programs within their city, but currently have very different intake processes, some only in paper form. They need a streamlined mobile intake app that collects essential information from an applicant, identifies the best program for the applicant and sends that applicant’s info to the appropriate organization.

UX Discovery

An initial Form Assembly survey was sent out to each organization inquiring how they currently process applicants. Organizations uploaded their current intake form at that time. Respondents were given the opportunity to voice what they like/dislike about their current intake process and what (if any) barriers they think might make a common intake process difficult. (excerpts shown below)

Discovery Insights

Each organization in the collective has multiple funding sources. Each funding source has specific requirements for what information is collected and kept on file for the people the organization serves. While each organization’s application has unique questions, there were 25 questions that 50% or more of organizations asked applicants on their intake form.  

These 25 questions were grouped into the following categories:​

1. Contact Info

2. Demographic Info

3. Household Info

4. Participation Interest and Availability

5. Referral and Consent

MVP Prototype

A MVP prototype was created in Form Assembly. This prototype will be presented to the collective over Zoom using Keynote to show options for wording on certain questions and get feedback on what questions need to stay or go from the form. (excerpts shown below)
